OPSF Employment Application and Required Documents

1. Download and fill out the full application using the link above.
2. These  documents  are included  in  the  above:
   1. Criminal History Request
   2. Health Information Report
   3. Employment Eligibility Verification
   4. Permission to conduct drug and/or alcohol testing
   5. Report of Contact with Law Enforcement
   6. OPD Criminal History Record Request
3. You can deliver the documents in person at our office at 3861 Farnam St, Omaha NE 68131 or email this to: Sarah.Dragon1@ops.org.
4. Schedule a Fingerprint Appointment on line and select Child Daycare Employee and contact us when this done. The contact information is include below.
5. We will be in contact with you when your criminal record results return to OPSF and we will let youknow if you are clear to work at Kids Club or Pre-K.
6. We will be emailing you instructions to complete the following:
   1. Central Registry (DHHS)
   2. Prepare to Care Training (DHHS)
   3. CPR Training (ProTraining)
7. Complete the Employment Application below and submit it online.
